P.O.P.P PROGRAM

RESIG POST OFFER/PRE-PLACEMENT
PROCEDURES FOR 2004-05 PROGRAM


1. Modify existing job announcements for the positions of: Bus Driver, Custodian, Delivery Driver, Food Service Worker*, Grounds Worker, Maintenance Worker, Special Day Class Teacher*, and Special Education Assistant*, to indicate that a Post Offer/Pre-Placement Physical Examination is required prior to hire. A copy of the physical demands job description should be provided to the applicant as part of the application packet so they are made aware of the physical requirements of the job.

*NOTE: The Physical Demands for Special Day Class Teacher, Special Education Assistant and Food Service Worker have been divided into two levels; Level I=High Physical Demands (i.e., Full Inclusion Assistants, Adult Education positions ,Food Service Workers who must also load and unload food delivery vehicles, etc.) and Level II=Moderate Physical Demands (i.e., Selpa Pre-School program, Elementary program, regular cafeteria workers, etc.). The District will determine what level is appropriate based on the lifting requirements and/or level of physical interaction with the students in the case of Special Education.

2. The Applicant is offered the position - contingent upon determination of fitness for the position. The District fills out the top portion of the District Follow Up Form and keeps the form on file until the screening has been completed.

3. The District is responsible for making the appointment with North Bay Corporate Health Services. This year as in last year's program, NBCHS is the PRIMARY PROVIDER and**Kaiser is only to be used as a back up provider in the event that NBCHS cannot accommodate an appt. within a reasonable amount of time. Both NBCHS and Kaiser have facilities in Santa Rosa and Petaluma. The appointment should be scheduled immediately after the applicant has been offered the position to reduce delays. Average turnaround time to schedule the medical screening should be between 3-5 days. For those choosing Kaiser, provide them with a questionnaire to fill out. They should bring the completed form with them to the appointment.

Note: Applicants do not have to be current Kaiser patients to use Kaiser services. However, if they are not current patients they will need to fill out a "Kaiser on the job" form to acquire a Kaiser medical record number. Fax this form to Kaiser (707) 571-4363 before an appointment can be made.

4. District will provide applicant with the Applicant Referral Form (½ sheet) to take with them to NBCHS/KAISER for appointment. (If appointment is with Kaiser, a copy of the Kaiser questionnaire should also be provided.)

5. NBCHS/KAISER will contact the District with the results of the screening, usually via fax. Generally, this will take place the same day as the screening unless there are complications requiring additional information or tests. There are some times when providers will hold results pending medical records. NBCHS will notify RESIG and RESIG will notify the District of any applicants placed on hold. After this notification, the District should contact the applicant to inform them of the reason for the hold and encourage them to contact their doctor to speed up the process. If the necessary records are not received within 30 days, applicant will be determined to be UNFIT.

6. Upon receiving results from NBCHS/KAISER, District fills out remainder of the District Follow Up Form indicating the results of the screening and hiring status and returns the form to RESIG. District may retain a copy for their files. Forms must be returned to RESIG within 30 days. Any forms not received within 60 days may require the district to reimburse RESIG the cost of screening.

7. IMPORTANT NOTE: If an applicant is found to be NOT FIT FOR THE POSITION (i.e., Unfit or Unfit at this time, Disqualified, screened for one category, but hired in another category with higher physical demands that is covered by the program or not being screened at all, and is HIRED by the District, there is a $12,000 deductible for any workers' compensation claim that is filed by that individual (for any reason) during their employment with the District.

8. NOTE: Per SCOE legal, the summary sheets from providers are to be treated as medical records and should be kept separate from regular personnel files.

                                       KEY ELEMENTS

1. THE 2004-05 PROGRAM IS MANDATORY FOR ALL MEMBER     DISTRICTS.

2. MEDICAL SCREENING IS REQUIRED FOR ALL POST OFFER     APPLICANTS FOR THE POSITIONS OF:
          •Bus Driver
          •Custodian
          • Food Service Worker - NEW Level I and Level II this year!!
               Level I - High Physical Demands (including loading and unloading                delivery vehicles)
               Level II - Moderate Physical Demands
          •Grounds Worker
          •Maintenance Worker
          •Special Day Class Teacher
               Level I - High Lifting Requirements
               Level II - Moderate Lifting Requirements
          •Special Education Assistant
               Level I - High Lifting Requirements
               Level II - Moderate Lifting Requirements

The medical screenings are based on the physical demands job descriptions that were developed for each of the above job categories. All job announcements should be modified to reflect that a medical screening is required and will be based on the physical demands of each job.

This program does not apply to existing employees. The program applies only to those applicants for positions in the seven covered categories who have been offered the position contingent upon successfully completing the medical screening.

3. THE DISTRICT WILL BE SUBJECT TO A $12,000 DEDUCTIBLE FOR ANY WORKERS' COMPENSATION CLAIM THAT IS FILED BY AN INDIVIDUAL WHO WAS FOUND UNFIT, UNFIT AT THIS TIME OR DISQUALIFIED DURING THE SCREENING, BUT WAS HIRED. THIS $12,000 DEDUCTIBLE ALSO APPLIES TO CLAIMS FILED BY ANY INDIVIDUAL IN THE ABOVE JOB CATEGORIES WHO WAS HIRED, BUT DID NOT GO THROUGH THE POPP SCREENING PROCESS OR WAS SCREENED FOR ONE COVERED JOB CATEGORY, BUT THEN WAS HIRED INTO ANOTHER JOB COVERED UNDER THE PROGRAM WITH HIGHER PHYSICAL DEMANDS.
